To prepare a Government ID:
Please have your Division Administrator or Supervisor email Tamra Eubank with a request for a Government ID for you.
In this email include the following information:
- Name (as you would like it to appear on the card)
- Your Job Title
- Social Security Number (This is optional, I recommend leaving it as N/A)
- Birth Date
- Height
- Weight
- Hair color
- Eye Color
- Travel Dates
Please double check the information for accuracy.
